- From the Sign In page, click Forgot Password.
- Choose how you would like to reset your password by clicking one of the three options.
- Type in the information requested.
- Click Submit.
A password reset link will be sent to the email address(es) associated with this account. Follow the instructions in the email message to change your password.
Email Address Not Valid
If you are a provider and the email address you are using is not recognized, please refer to the Account Access FAQ.
If you are a LANE or QIO and are unable to obtain your username/password, please email the Campaign Help Desk with your request along with state, name, and organization.
