Advance Care Planning (ACP)
Care planning should always include a discussion with your resident about their preferences for care in the event that they have a decline in function or ability to communicate.The Advance Care Plan does not address only care at the end of life, but documents an overall vision for activities, accommodations, adaptations, and care during a gradual decline in function/ability to communicate.
